How often does the filter need to be replaced?

The filter doesn't need to be replaced, it is made of 304 stainless steel, you can use it for a long time.

Does the spout remain in the decanter for pouring? I’m looking for a decanter that doesn’t drip when pouring so hoping the “spout” stays in after de.

Yes, you leave the spout in the decanter. It pours very nicely and does not drip. My parents use it everyday, and they serve from decanter — best gift EVER!

Does this come with the scrubber in the picture?

Mine did come with the scrubber. Very useful!

In the winter there are some wines I like to drink warm. Wondering if the bottom is flat so it can be placed on an electric cup warmer?

It is not flat and the glass is thin. I'm not sure it would do well on a hot burner/warmer
